To register for Summer Swim:

1. Fill out the form below and submit for each participant.

2. Bring a check for the appropriate amount made to "City Aquatics" to the pool or tri-club practices.

All fees and/or request for fee assistance or waivers are due by June 1st.

3. Please note that registration for the South Shore or McDoanald's Kids triathlons is not included. We will have registration forms at practice or register online at www.tri-masters.org or www.chicagotriathlon.com .
